 (Reminder: The original documents of items#1, #7, #8, #11, #12, and #9,#14~#16 (if applicable) , the certificate/ license and training records of #17  (if applicable) etc. need to be presented at the time of the audit. 注意：在審核當(dāng)日必須提供審核員以下#1、#7、#8、#11、 #12項(xiàng), 以及#9、 #14~#16項(xiàng)(如適用) , 還有#17，#18 所述的證明/培訓(xùn)記錄（如適用）的各項(xiàng)文件之原件).

3. Organization Chart (組織機(jī)構(gòu)圖)

4. Factory rules and regulations  (工廠廠規(guī)廠紀(jì))

5. Dormitory rules and regulations  (宿舍紀(jì)律規(guī)則)

6. Employee handbook (員工手冊(cè))

7. Individual Labor Contract is required; Group labor contract is allowed as long as government’s written document provided and has clear traceability.(需要提供個(gè)人勞動(dòng)合同，清晰并且通過政府認(rèn)可的集體合同可以接受)

8. Personnel file 

·   ID copy  (身份證復(fù)印件)

· Application with joining date  (人事檔案資料-包括入廠日期)

· Leave application  (請(qǐng)假單)

· Resignation letter / record  (離職信/記錄)

· Rewards and punishment record (獎(jiǎng)懲記錄)

9. Health examination report and registration for workers between 16 to 18 years old. (16-18歲未成年工體檢報(bào)告及上崗證/登記)

10. Local official minimum wage document(當(dāng)?shù)卣�府關(guān)于最低工資的文件)

11. Payroll records and production records for past 12 months: (最近一年的工資 / 生產(chǎn)記錄)
